A New Model of Primary Health Care. In 2007, the Ontario Ministry of Health and Long-Term Care announced the creation of 25 Nurse Practitioner-Led Clinics. The Glengarry Nurse-Practitioner Led Clinic opened on February 28, 2011, and was the fifth of its kind to open in the province. Nurse Practitioner-Led Clinics are a new primary health care delivery model in which Nurse Practitioners are the lead providers of primary health care, and we are governed by a not-for-profit Board of Directors.
